Facilities Request Form
Attention: Clearance for facility reservations is granted based on facility availability, structured priority and access guidelines, appropriateness of activity and advanced notice. When completing this form please provide ALL the information requested including secondary choices for times and dates. REQUESTS WILL ONLY BE ADDRESSED IF RECEIVED TWO WEEKS PRIOR TO THE EVENT. You will be contacted soon after submission as to the status of the request. A permit will be issued if your request is granted and a meeting date will be set prior to the event. Be advised that at times, permitted requests may be changed due to priority groups. Thank you.

As we prepare to begin allowing student organizations to reserve indoor spaces for programming/activity, there is important information for the Temple community to ensure healthy and successful events. Please note that max occupency numbers have been set based on space for all indoor event/activity requests.
Temple currently requires that all members of the campus community wear masks indoors and in enclosed spaces.
Campus Recreation reserves the right to decline any indoor events or activities including but not limited to the following:
- Indoor Soccer
- Contact sports
- High intensity activities causing contact
